The patient portal is a secure website that helps you manage and receive information regarding your care at this practice. The portal allows you to communicate with us easily and safely - according to your schedule. Through it, you can:
- Access your personal health information
- View test and lab results
- Request appointments and prescription refills
- Receive important reminders about your care
- Exchange messages with the practice
- Update personal and insurance information
- Complete and update medical forms
- Set up proxy accounts for children and dependent adults

Log-in requires an invitation code, which our staff provides after verifying your photo identification. If someone has not offered you the opportunity to register and receive an invitation code, please let us know. Or, if you would like more information, just ask. We encourage you to sign up today!
Steps to log in and utilize the portal:
- Click the registration link in the email you receive from
- Create your portal login using a unique username and password.
- Enter the invitation code provided by your medical organization and click "Agree" to the release of information.
- You will receive several confirmation emails including one welcoming you to FollowMyHealth and one confirming your connections with Heart Center of Southern Arizona.
